Security and Privacy Incident Reporting

If you suspect that a security or privacy incident has occurred, please contain the incident before reporting it to Brain-CODE, if possible.
All security and privacy incidents must be reported to Brain-CODE for review by the Information Security Committee (ISC).
To report an incident, please email alanasparks@indocresearch.org or emartens@indocresearch.org.
Please have the following information ready:
Contact information of the person who discovered the incident
Contact information of the person who first reported the incident
Any applicable supporting documentation
Date/time the incident occurred
Date/time the incident was discovered
Date/time the incident was reported
Systems affected by the incident
Your program and site (if applicable)
Incident description
Number of participants affects
Systems affected
Cause of the incident
Summary of immediate containment steps
