If you suspect that a security or privacy incident has occurred, please contain the incident before reporting it to Brain-CODE.

All security and privacy incidents must be reported to Brain-CODE for review by the Information Security Committee (ISC).

To report an incident, please complete the Security Incident Reporting Form.

Please have the following information ready:

  • Contact information of the person who discovered the incident

  • Contact information of the person who first reported the incident

  • Any applicable supporting documentation

  • Date/time the incident occurred

  • Date/time the incident was discovered

  • Date/time the incident was reported

  • Systems affected by the incident

  • Your program and site (if applicable)

  • Incident description

  • Number of participants affects

  • Systems affected

  • Cause of the incident

  • Summary of immediate containment steps
